Book your appointment online, call us at 718-885-4255, or email us as mysmilespeds@gmail.com. We are happy to assist you with any of your scheduling or emergent dental needs.
New Patient Forms
Once you've booked your appointment, you will receive a confirmation email for us with intake forms - save that number as we can communicate with each other via text through that office mobile number! Please also email us a front and back copy of the insurance card. Once the forms are filled out they will automatically upload into our system so no need to print them or waste precious appointment time filling out forms.
Payment options
We work with many major PPO dental insurance plans as a form of payment. We accept Visa, MasterCard and American Express. We will gladly verify your insurance before your arrival and submit claims on your behalf.
